Welcome to the Practice! Before your child's first visit with us, please look over this section for information about our hospitals, insurance, billing, and the registration forms that you can complete and bring in with you, mail, or fax to us to facilitate your visit. We make every attempt to confirm your appointment by phone a day ahead. Please remember to keep you phone contact information current. If your child is under 18 years of age he or she must be accompanied by a parent or guardian. A permission slip with parent signature is required if your child is being brought to his or her appointment by a relative, sitter, or someone other than the parent.
If you provide us with your email address, we can officially invite you to join Relay Health or you can register online at www.relayhealth.com This is a service that allows you to request appointments and prescription refills via email. Currently, our clinicians are not using email to directly communicate with parents.
